2025-2030 Community Health Improvement Plan Dashboard
Overview
A Community Health Improvement Plan (CHIP) establishes priorities and guides efforts and strategies to improve public health.
Sullivan County’s 2025-2030 CHIP was informed by the Regional Community Health Assessment (CHA). Sullivan County worked with the six other Mid-Hudson counties (Dutchess, Orange, Putnam, Rockland, Ulster and Westchester) to assess community health through data collection and analysis. This dashboard highlights data objectives for the following three priorities:
- Access to and Use of Prenatal Care
- Preventive Services for Chronic Disease Prevention and Control (Asthma)
- Healthy Children - Preventive Services (Immunization)

Priority Summary
NYS Prevention Agenda Statewide Objective: Increase the percentage of birthing persons who receive prenatal care during the first trimester from 80.7% to 83.0%.
Higher rates are more desirable. In 2023, the percentage in Sullivan County was 58.5%, lower than the other regions and a decrease since 2019.
Action Plan
With programs such as Healthy Families, and partnership with local providers, high schools, and SUNY Sullivan, SCDPH aims to
- Connect pregnant people with care providers and health insurance
- Increase awareness of early prenatal care through outreach and social media
- Provide education and support to pregnant people

Priority Summary
NYS Prevention Agenda Statewide Objective: Decrease the asthma emergency department visit rate per 10,000 among children aged 0-17 years from 93.8 to 89.1.
Lower rates are more desirable. In 2024, the rate in Sullivan County was 112.6 per 10,000 children aged 0-17 years. This is higher than the other regions and an increase from 2019.
Action Plan
With programs such as Children and Youth with Special Health Care Needs (CYSHCN), local providers, and schools, SCDPH aims to
- Make sure providers can offer asthma action plans that are accessible and in patients’ languages
- Provide evidence-based asthma education to families
- Provide referrals to appropriate care for asthma

Priority Summary
NYS Prevention Agenda Statewide Objective: Increase the up to date seven-vaccine immunization rate for children 24-35 months from 59.3% to 62.3%.
Higher rates are more desirable. In 2024, the percentage in Sullivan County was 48.1%, lower than the other regions. The lowest rates are seen in the years 2021 and 2022.
Action Plan
Through the SCDPH Diagnostic and Treatment Center and partnerships with schools and other Mid-Hudson counties, SCDPH aims to
- Increase awareness of access to immunization regardless of insurance coverage
- Increase the number of mobile immunization clinics offered
- Provide education to families and prevent student exclusion from school
